This Settlement resolves a class action lawsuit against JPMorgan Chase & Co. and JPMorgan Chase Bank, N.A. (?Chase?) involving a Post Closing Fee or Post Closing Review Fee (referred to herein as ?Post Closing Fee?) charged by Chase on some mortgages it originated between September 22, 2001 and December 31, 2008. Under the Settlement, you are eligible to receive a refund of the Post Closing Fee you paid to Chase.
In order to receive a refund, you must fill out a claim form. You can download a claim form here.
If you requested a copy of your Settlement Statement from the Claims Administrator, please note the date you requested the Settlement Statement on line E and submit your claim form. You will not be sent a copy of your Settlement Statement. The Claims Administrator will obtain the Settlement Statement and process your claim.

I received a claims form from chase and sent it back months ago and have not received anything
